Pragmatic Free Trial Meta
Pragmatic Free Trail Meta is an open data platform that facilitates research into pragmatic trials. It is a platform that collects and shares clean trial data and ratings using PRECIS-2, permitting multiple and varied meta-epidemiological research studies to examine the effects of treatment across trials that employ different levels of pragmatism, as well as other design features.
Background
Pragmatic studies are increasingly recognized as providing real-world evidence for

The purpose of pragmatic trials is to guide the practice of clinical medicine and policy choices, rather than prove a physiological or clinical hypothesis. A pragmatic trial should aim to be as close as possible to real-world clinical practices that include recruiting participants, setting up, delivery and execution of interventions, determination and analysis outcomes, and primary analysis. Studies that are truly practical should avoid attempting to blind participants or clinicians, as this may lead to distortions in estimates of treatment effects. Pragmatic trials should also seek to enroll patients from a wide range of health care settings so that their results can be compared to the real world.
Finally the focus of pragmatic trials should be on outcomes that are crucial for patients, such as quality of life or functional recovery. This is particularly important for trials involving the use of invasive procedures or potential serious adverse events. The CRASH trial29,

The creation of a PRECIS-2 tool that can provide an objective, standardized assessment of pragmatic features is a good start.
Methods
In a practical study, the goal is to inform policy or clinical decisions by showing how an intervention can be integrated into routine care in real-world contexts. Explanatory trials test hypotheses about the causal-effect relationship in idealized environments. In this way,

The PRECIS-2 tool scores an RCT on 9 domains, with scores ranging between 1 and 5 (very pragmatist). In this study, the recruit-ment organization, flexibility in delivery and follow-up domains received high scores, however the primary outcome and the method of missing data fell below the pragmatic limit. This suggests that it is possible to design a trial with excellent pragmatic features without harming the quality of the results.
It is hard to determine the level of pragmatism within a specific study because pragmatism is not a have a single attribute. Certain aspects of a study can be more pragmatic than others. Additionally, logistical or protocol modifications made during the trial may alter its pragmatism score. Koppenaal and colleagues found that 36% of the 89 pragmatic studies were placebo-controlled, or conducted prior to licensing. They also found that the majority were single-center. Thus, they are not as common and can only be called pragmatic in the event that their sponsors are supportive of the lack of blinding in these trials.
Furthermore, a common feature of pragmatic trials is that the researchers attempt to make their findings more meaningful by analysing subgroups of the trial sample. However, this can lead to unbalanced comparisons and lower statistical power, thereby increasing the chance of not or
프라그마틱 순위 incorrectly detecting differences in the primary outcome. This was a problem during the meta-analysis of pragmatic trials due to the fact that secondary outcomes were not corrected for differences in covariates at baseline.
Furthermore practical trials can have challenges with respect to the collection and interpretation of safety data. It is essential to increase the accuracy and quality of the outcomes in these trials.
Results
While the definition of pragmatism may not mean that trials must be 100% pragmatic, there are benefits of including pragmatic elements in clinical trials. These include:
Increasing sensitivity to real-world issues as well as reducing cost and size of the study as well as allowing trial results to be more quickly transferred into real-world clinical practice (by including patients who are routinely treated). Schwartz and Lellouch1 developed a framework to differentiate between explanation studies that support the physiological hypothesis or clinical hypothesis and pragmatic studies that guide the selection of appropriate treatments in the real-world clinical practice. The framework was comprised of nine domains scored on a 1-5 scale, with 1 being more explanatory while 5 was more pragmatic. The domains included recruitment and setting, delivery of intervention, flexible adherence, follow-up and primary analysis.
The original PRECIS tool3 was built on the same scale and domains. Koppenaal et al10 devised an adaptation of this assessment dubbed the Pragmascope which was more user-friendly to use in systematic reviews. They discovered that pragmatic reviews scored higher in all domains, but scored lower in the primary analysis domain.
The difference in the primary analysis domains could be due to the way in which most pragmatic trials approach data. Certain explanatory trials however don't. The overall score for systematic reviews that were pragmatic was lower when the domains of organization, flexible delivery, and follow-up were merged.
It is crucial to keep in mind that a pragmatic study should not mean a low-quality trial. In fact, there is increasing numbers of clinical trials which use the word 'pragmatic,' either in their abstract or title (as defined by MEDLINE, but that is neither precise nor sensitive).
