Many serious accidents have happened due to a machine being started when it should have been shut off to allow for maintenance or repair. These accidents are usually the result of energy that was not released properly. Lock-out-tag-out (LOTO) that ensures that machines are shut off until a designated individual releases them, helps avoid these accidents.

The LOTO system is comprised of a series of locks, tags and other devices that are used to isolate, secure or shut off the machine from electrical sources. All "affected", "authorized", and "qualified" personnel should be instructed on the use of lockout and tagout devices. These devices include, but are not only restricted to chains, locks, and padlocks. Each device is identified and labeled, including the date and time it was applied along with the name and address of the person who was authorized to use it, and the purpose of the lockout or tagout (repairs, maintenance, etc.). It should also be clearly stated that these devices are not able to be removed until an entire LOTO procedure has been completed.
